McKenzie Method Assessment and Treatment
One of the benefits of the McKenzie Method (or McKenzie Therapy) is that it is a standardized approach to both the assessment and treatment of low back pain and/or leg pain (sciatica). The McKenzie Method is not simply a set of exercises; it also classifies spinal problems so they can be adequately treated.
The McKenzie Method is grounded in finding a cause and effect relationship between the positions you usually assume while sitting, standing, or moving, and the generation of pain as a result of those positions or activities. The therapeutic approach requires you to move through a series of activities and test movements to gauge your pain response. We then use that information to develop an exercise protocol designed to centralize or alleviate the pain.
The Doctors at Merrick chiropractic will assess what activities, postures, and motions are causing your specific pain by the information they obtain from your health history and examination. The McKenzie method breaks down spinal disorders to 3 categories.
The Postural Syndrome-
This syndrome involves pain produced by prolonged end range mechanical stress on normal structures,which is immediately relieved by reduction of the stress. This is not seen too much because the pain can be relieved by the patient changing his position.
The Dysfunction Syndrome
This is pain produced by end range stress on shortened structures such as scar tissue or adhesions. Pain is only produced at end ranges and repeated movements will not increase the pain.
The Derangement Syndrome
This is an anatomical disruption or displacement within the motion segment. Symptoms may be produced at mid or end ranges and the symptoms may increase(centralize) or decrease(peripheralize). Deformities may accompany derangments. These may take the form of compensation to relieve weight-bearing stress on injured or inflamed tissue. It may also be that displaced material(slipped disc) is physically blocking the motion segment. Unlike the other 2 syndromes the symptoms and physical findings may be changing rapidly. Range of motion can dramatically increase or decrease with position,activity and pain intensity and location of pain can increase or decrease.
This refers to the most distal symptoms(such as sciatic leg pain)moving toward the midline. When we treat derangments, centralization is the key indicator in choosing types of treatment. The focus of treatment is determined by the what body movement causes pain. So this type of treatment would involve exercises in pain free ranges as well as postural correction. Centralizing the pain correlates with long term functional relief. So the better the exercises work at centralizing pain, the less pain you will have and the better long term relief you will experience. An example of this is when you have leg pain from sciatic nerve pressure the exercise will cause the leg pain to lessen until it is just located in the lower back.
